Referee Forms

Referees and Assistant Referees are required to report all incidents that result in a yellow card, red card, referee abuse, serious injury or if a blue card has been issued to a player with suspected concussion.

These forms are required to the WRU within 48 hours of completion of the match for judicial purposes. By submitting the online form below the WRU Referee Manager & Administrator receive this form. They will contact you if your incident requires further information, or if there will be a judicial hearing for your incident.

If you require assistance completing a form contact a premier referee or WRU Game Developer Match Officals & Compliance officer Andrew Bowe. To view some examples of completed forms click on the document below.

Judicary Notes

Over recent seasons their has been a decline in red card incidents now going to a formal judicial meeting. A red card now carries an automatic two week suspension for the player, unless either the red carded player or their team want to appeal the two suspension, or if the independent judicial panel believe the incident requires a longer suspension.

In the case of a judicial hearing you will be contacted by Waikato Rugby's Game Developer of Match Officals & Compliance inviting you to the judicial hearing meeting where you will be provided an opportunity to share your statement on the incident. You will also be able to bring along a support person with you to this judicial meeting.
